I used a contact form that was in one of the demo sites to reverse engineer it. Sorry I did not have time to document the process, as it was some trial and error. However, I was able to get it working fine on my custom installation after following the steps that roughly included:
-Creating a new class called contact form with information collection on the appropriate datatypes (modeled this after the form in the demo site)
-Setting the email receiver in the site.ini.append
-modifying the template until it displayed properly

Overall, it was not as hard as I had suspected it would be. Now my editors can add a contact form anywhere on the site and change the title and the default information. The only problem is that they all email the same address with the results. There is a way around this, but I am not that good yet.
