Is the data collected properly in the database? If yes please check what templates are loaded for sending the email. To see those, empty the cache, set debugredirection and list used templates to enabled and submit your infocollection form. You will now see an redirection page which lists the templates used for creating the email. Check those templates and compare them to the templates found in \design\standard\templates\content\datatype\result\info
I have tried these templates using the same datatypes as in your form and they work fine for me.
