hmm, normally upload_images.tpl should have been used for images.
Do you have any specific ezoe.ini or content.ini[RelationGroupSettings] related overrides somewhere?

I found the solution this orning after reading this: http://share.ez.no/forums/setup-design/sol-ezoe-unable-to-embed-images-after-upgrade-to-4.1.1
I had the exact same issue, adding [small] to my image.ini (I had removed it) override fixed it.
