I've never seen this problem, but I'm interested in any information so we will be able to re produce it as well.
Could this be related to people doubleclicking when they submit the account information? (I don't think so, but it might be a cause)

I know this is a very old topic, but I encounter the same problem. Apparently any validation of attributes is bypassed when cookies are disabled in the user agent, and user/register published the user object.
