Two things are wrong. 1. eZ publish 3.7 only works with PHP 4.4.x, use 3.6 for PHP 4.3.x2. The survey module does not work with eZP3.7/PHP4.4.x
Easiest solution in your case might be to use eZ publish 3.6 instead.

With some fixes the survey module works with eZ publish 3.7/ PHP 4.4. See the comments on the contribution page.
