This just happened to me but the solution was a chmod. Try chmod higher up in the directory structure. Try a recursive '777' if all else fails.

I did the usual ./bin/modfix.sh, but as the folders in question are owened by user:usersgroup and the chomding gives 770, poor litle apache wasn't allowed to go there...

i am experiencing the same problem. after an upgrade from 3.2.4 to 3.4.4 no images are visible anymore. the old images are still in their directory structure, in the folder "original" and "reference". new image get uploaded OK into the new directory structure and also show up perfectely.

when i look at the HTML output, then i see, that there is no image or image path in the HTML IMG command. it simply is IMG="".

looking at debug output, it tells me, that CHMOD is not possible. that could be due to the fact, that my provider does a security upgrade and might have disabled php exec. my images are created with GD.

the whole var directory and everything under it has rights 777.

any ideas???
