The same is the case with me. Perhaps the ezapprove event type simply isn't set up to send out emails after creating collaboration items (which I would fing strange)?
Can someone from eZ please confirm or deny this?

You need to run the cron to get the emails on the workflow.
Do you run it ? (for the tests, you can run it from the cli, and disable several tasks, like testing if the urls are valids, it takes ages).
