Design, base and standard are just layout that override each other. You can set the priority in settings/override/site.ini.php.append
To customize a layout you can change the css or edit the template (in admin panel, through design / template).
Pagelayout.tpl is overall page frame while content is generated by the node/view/full or node/view/line override.
