Hint1: Look at the overrides used by the base siteaccess.
Hint2: Take a look at the collect.ini file.
Hint3: You could just create a design/your_design/collectedinfo/form.tpl, clear the caches and it would be loaded instead of the template that is in the standard design.
This are the override rules i used:
[collected_feedback_form]
Source=content/collectedinfo/form.tpl
MatchFile=collectedinfo/feedbackform.tpl
Subdir=templates Match[class]= '16'

Well, i created design/shop/override/templates/collectedinfo/feedback_form.tpl, but its not working. Do i really need to clear the caches?
